1. Whistleblowers shall submit a report to the Compliance Officer using one of the following channels, at their discretion:
a) By email, by sending the report to: sygnalisci@apsprofit.pl The submission form is available on the company’s website at this address.
b) By post, by sending a written report to the Compliance Officer, 14B/U1 Emilii Plater Street, 71-635 Szczecin, with the note on the envelope: Confidential.
c) In writing, by delivering the report in a sealed envelope to the Compliance Officer.
d) In person, by meeting with the Compliance Officer.
2. The notification should include at least:
a) an indication of the type of legal violation that is the subject of the notification,
b) the date and place of the occurrence of the violation or the date and place of obtaining information about it,
c) a description of the specific situation or circumstances giving rise to the possibility of the violation,
d) an indication of the person to whom the notification relates,
e) an indication of possible witnesses to the violation, as well as all evidence and information available to the whistleblower that may be helpful in the process of reviewing the notification,
f) the whistleblower’s name and contact details (correspondence address, email address, telephone number),
g) an indication of position or function within the company,
h) the whistleblower’s legible signature.
3. The whistleblower’s personal data and other information allowing for their identification shall not be disclosed, except with their express consent.
4. The whistleblower may use the submission form available at this address.
5. Anonymous submissions will not be accepted.
6. The Compliance Officer shall provide the whistleblower with an acknowledgement of receipt of the notification within 7 days of receiving it, provided that a contact address was included in the notification, enabling confirmation to be sent. The receipt of the notification is understood as the moment it reaches the Compliance Officer through one of the channels indicated in § 4(1) of the Regulations, in a manner that allows for its content to be reviewed.
7. Annexes
